1. On 12/16/2021, Title II ADA / Section 504 Coordinator(s) from the Office of Accessibility performed a Title II - Americans with Disabilities Act (ADA) Self-Evaluation. This evaluation was performed in accordance with 1991/2010 ADA Standards for Accessible Design.
2. Priorities for removing barriers are assigned in accordance with the ADA Checklist for Existing Facilities, promulgated by the ADA National Network. The checklist follows the four priorities listed in the Department of Justice ADA Title III regulations. These priorities are equally applicable to state and local government facilities:
Priority 1 - Accessible approach and entrance
Priority 2 - Access to goods and services
Priority 3 - Access to public toilet rooms
Priority 4 - Access to other items such as water fountains and public telephones
3. Corrective actions to findings were / will be submitted to the City maintenance work request system and will be managed in accordance with the City Transition Plan. Note: Recommended corrections may be modified where appropriate. Corrections are developed in consideration of the “Program Access” provisions of Title II, where applicable.

Running slope is excessive on 14 pads measuring between 5.2% to 9.6%. Cross-slope is between 2.3% and 6.7% on 44 pads. 2 pads exceed 8.33% running slope at 9.6% each, 26 pads exceed 3%.
Recommendation:
Remove and replace pads to compliance. Regrade the path to achieve a compliant running slope which appears to be feasible given the existing terrain.

From the turn to the basketball court up to the turn to the first playground there are 14 pads of concrete with excessive cross slope and/or running slope. Cross slope ranges from 2.3% to 7.9% on eight pads while running slope is between 7.2% and 12.3% on 12 pads. On 7 of the 9 pads, the cross slope exceeds 3%. On 8 of the 12 pads, the running slope exceeds 8.33%.
Recommendation:
Remove and replace pads with high cross slope. It might not be feasible to achieve a compliant running slope due to the existing terrain. Explore options of zig-zagging the path in order to achieve a more gradual ascent of less than 5% up to the playground. If a route is created to the volleyball court, it may be possible to achieve compliance by creating a route from the volleyball court to the playground.

Recommendation:
Leave this section intact as nothing can be done to the existing route to achieve a compliant running slope. Explore adding a connection between the two playgrounds in order to achieve a compliant running slope.

The section of the accessible route that runs from the turn to the south playground to the south end of the park exiting into the residential area has 46 pads of concrete with excessive running slope and/or cross slope. Running slope measures between 5.4% and 9.5% on 27 pads, while cross slope has a range of 2.6% to 11.1% on 39 pads of concrete. 3 out of the high running slope pads exceeds 8.33%. 34 of the 39 high cross slope pads exceeds 3%.
Recommendation:
