1. On 11/24/2021, Title II ADA / Section 504 Coordinator(s) from the Office of Accessibility performed a Title II - Americans with Disabilities Act (ADA) Self-Evaluation. This evaluation was performed in accordance with 1991/2010 ADA Standards for Accessible Design.
2. Priorities for removing barriers are assigned in accordance with the ADA Checklist for Existing Facilities, promulgated by the ADA National Network. The checklist follows the four priorities listed in the Department of Justice ADA Title III regulations. These priorities are equally applicable to state and local government facilities:
Priority 1 - Accessible approach and entrance
Priority 2 - Access to goods and services
Priority 3 - Access to public toilet rooms
Priority 4 - Access to other items such as water fountains and public telephones
3. Corrective actions to findings were / will be submitted to the City maintenance work request system and will be managed in accordance with the City Transition Plan. Note: Recommended corrections may be modified where appropriate. Corrections are developed in consideration of the “Program Access” provisions of Title II, where applicable.

The sidewalk down Cracker Barrel Cir has 51 pads of concrete with excessive cross-slope measuring as high as 6%. This stretch of sidewalk also has a high running slope throughout, however, it follows street grade and therefore meets compliance. 29 pads are at or exceed 3% cross slope.
Recommendation:
Remove and replace pads with high cross slope to compliance.

The path into the park, just north of the baseball field, that connects to the Homestead Trail in the northeast corner of the park, has 31 pads of concrete with excessive cross slope and/or running slope. There are 29 pads with a high cross slope measuring up to 5.2%. 14 of the pads are at or over 3% cross slope. There are 13 pads with a high running slope of up to 9%. 4 pads exceed 8.33%.
Recommendation:
Remove and replace the pads with a high cross slope. The terrain is such that it may not be feasible to achieve a compliant running slope.
