1. On 06/09/2022, Title II ADA / Section 504 Coordinator(s) from the Office of Accessibility performed a Title II - Americans with Disabilities Act (ADA) Self-Evaluation. This evaluation was performed in accordance with 1991/2010 ADA Standards for Accessible Design.
2. Priorities for removing barriers are assigned in accordance with the ADA Checklist for Existing Facilities, promulgated by the ADA National Network. The checklist follows the four priorities listed in the Department of Justice ADA Title III regulations. These priorities are equally applicable to state and local government facilities:
Priority 1 - Accessible approach and entrance
Priority 2 - Access to goods and services
Priority 3 - Access to public toilet rooms
Priority 4 - Access to other items such as water fountains and public telephones
3. Corrective actions to findings were / will be submitted to the City maintenance work request system and will be managed in accordance with the City Transition Plan. Note: Recommended corrections may be modified where appropriate. Corrections are developed in consideration of the “Program Access” provisions of Title II, where applicable.

The triangular junction located at the far south end of the park near the Homestead Trail has the following issues:
- Turn A has a slope measuring 5.7% as well as gaps between pads of more than 1" wide and 2" deep. There are also cracks, breaks, and divots resulting in vertical displacement up to 3/4".
- Turn C has a slope measuring 2.9%.
- Route Section 1 has two pads with a high cross slope measuring 2.7% and 2.6%.
- Route section 2 has two pads with a high cross slope measuring 2.9% and 3.5% as well as a crack in one pad resulting in a 1/2" vertical displacement.
- Route Section 3 has 7 pads of concrete with a high cross slope measuring between 2.4% and 3.8% (2 exceed 3%).

The triangular junction located just east of the playground has the following issues:
- Turn A has a slope measuring 4.7%.
- Turn B has a slope measuring 8%.
- Turn C has a slope measuring 7.3% and is severely fractured.
- Route section 1 has 8 pads of concrete with a high running slope and or cross slope. The running slope measures between 5.4% and 8% on 7 pads, while the cross slope is between 2.8% and 3% on 3 pads.
- Route section 2 has 14 pads of concrete with a high cross slope measuring between 2.3% and 5.3% (10 pads are 3% or more). There is also a 3/4" vertical displacement with a 3/4" gap between pads.
- Route section 3 has 10 pads of concrete with a high cross slope and/or running slope. The Cross slope measures between 2.4% and 4.7% on 8 pads (5 exceed 3%), while the running slope is between 5.3% and 6.8% on 3 pads.
